Declaration

In giving consent, we understand and agree that any money held in the client money trust bank account of Trust Insurance Services Ltd as at the close of business on the 31 December 2021 may be transferred to the client money trust bank account of PSC UK Insurance Brokers Ltd (FRN 457721).

We also understand and agree that any money that is paid after 1 January 2022 to the client money trust bank account of Trust Insurance Services Ltd may be transferred to the client money trust bank account of PSC UK Insurance Brokers Ltd (FRN 457721).

I confirm that I have the authority to make this decision on behalf of the policyholder
